My name is Tim Gode. I was diagnosed with cystinuria about 12 years ago and have been a member of the cystinuria support group for about 3 years. Recently, I decided to conduct a study on ourselves that would answer questions that we, as a group, have about our condition. Many of us were sharing our experiences and providing alternatives to situations that we ran across. This is great but we can do more. A lot of questions have been put out to the group that no single one of us could answer. For instance, someone once posed the question that there might be a relationship between how we sleep and which kidney our stones form in most often. Many of us responded with our own experience. I form stones only in the right kidney and I toss and turn at night. Other people sleep on one side and wrote that they form stones in both kidneys. So what' the s answer? The only way to find out is to gather these facts on everyone and then see if the relationship exists. Someone else wondered if cystinurics might have a weight problem. The only way to truly answer this is to get everyone' weight, height, and age on a list and s see if the data shows our group to be higher than the average for the rest of the population. Another area we' discussed at length is side effects ve with our drugs. Many of us have experienced side effects with either the cystine binding drugs or painkillers we use during stone attacks. How does our experience with side effects compare to the medical literature? When the drug pamphlet says there is a 1 chance of a certain side effect occurring, is that what we' experienced? ve Since this information either does not exist or is not available to us, an organized study of ourselves is the only way we' get answers. A questionnaire was ll developed using input from all the on-line cystinuria support group members. The result was a form that probes into the lives, habits, and medical histories of, for example, disopyramide side effects.

Has been recently questioned. Its primary mechanism of action is through sodium and water retention at the expense of some potassium wasting. In addition, Fludrocortisone may aid in sensitizing alpha-receptors and blocking vasodilation. Fludrocortisone has modest, if any, corticosteroid side effects. A new, direct-acting alpha-1 agonist, midodrine proamatine, given as 5-15 mg dose spaced 4 hours apart, three times a day ; has been used to good effect in many patients with assorted forms of OI. This medication treats a deficit in norepinephrine-related vasoconstriction. Other interventions have included alpha-2 adrenergic agents, both clonidine and yohimbine, which have been used in select patients. Disopyraamide has been used occasionally, but controlled studies do not support its efficacy.228 Recently, selective serotonin reuptake inhibitors have been used to good effect in a variety of orthostatic disabilities. These seem to interfere with hypotensive responses at a central level. Grubb and associates have demonstrated efficacy of sertraline and fluoxetine in a series of controlled studies.242, 243 The studies were performed after careful psychiatric screening had ruled out significant depression. Personal experience bears this theory out and the SSRIs remain a useful form of therapy for many forms of OI. It remains unclear whether any treatment repairs any specific defect. Medical therapy must therefore continue to be regarded as palliative. Effective and specific treatment for chronic OI can only be developed when a specific etiology or etiologies are discovered. It seems likely that even when there is clinically diagnosed POTS, CFS remains heterogeneous with a variety of specific etiologies, giving rise to common clinical features. Conclusion The evidence is clear that OI plays a key role in the pathophysiology and symptomatology of patients with CFS. Recent scientific investigations have broadened the scope of orthostatic abnormalities to include a wide range of illness in which blood flow and blood pressure regulation are impaired. Several treatment modalities may improve the symptoms resulting from POTS associated with CFS.
CHILD NEURODEVELOPMENT FOLLOWING TREATMENT FOR NAUSEA AND VOMITING OF PREGNANCY WITH DICLECTIN: A PROSPECTIVE CONTROLLED STUDY. I. Nulman, MD, H. Illios, MA, D. Knittel-Keren, BA, M. Barrera, PhD, G. Koren, MD, The Hospital for Sick Children, Toronto, ON, Canada. BACKGROUND: Nausea and vomiting of pregnancy NVP ; occurs in 80% of women, may lead to Hyperemesis Gravidarum HG ; . The only antiemetic approved by Health Canada for NVP is Diclectin doxylamine and vitamin B6 ; . Diclectin does not cause fetal dysmorphology, its effects on the developing central nervous system CNS ; remains to be established. Potential adverse effects of the drug on fetal CNS may constitute a serious public health issue. OBJECTIVES: To assess whether children prospectively collected exposed in utero to NVP and Diclectin and tested with formal psychological tests are different from children exposed only to NVP, and from children not exposed to NVP, Diclectin, and other teratogens. METHODS: A prospective, controlled assessment 3 groups of mother-child pairs exposed to NVP and Diclectin n 41 ; , exposed to NVP n 37 ; and unexposed to NVP or teratogens n 30 ; . compared the neuro-cognitive outcomes, language, and measures of child behavior among the 3 groups. RESULTS: There were no significant differences between the groups in Global IQ percentile 79.4 20.5; 73.1 ; , total percentile of PLS-4 80.4 28.5; 73.9 ; and CPRS-R-L scores 48.4 7.7; 50.3 ; . CONCLUSIONS: Exposure to Diclectin does not adversely affect neuro-cognitive development of early school children. When indicated, Diclectin therapy should be instituted to prevent HG, and improve pregnant women life style. Supported by Duchesnay Inc., Laval, Canada and motilium, for instance, prednisone.

Objective: To determine if protein and energy requirements in the Regina Qu'Appelle Health Region RQHR ; Dietitian Handbook for pancreatitis, spinal cord injury, and multiple trauma are valid and evidence-based. Methods: A review of literature from 1975-2006 was conducted using MEDLINE. Inclusion criteria were date published 1975-2006 ; , primary research article, human adult subjects, applicable to disease state and nutrition energy and or protein requirements ; . Studies were critically evaluated and assigned a level and grade of evidence. The strengths and limitations of the studies were used to make recommendations. Results: No recommendations were made for pancreatitis. For acutely injured individuals with paraplegia and quadriplegia, it should be considered that energy could be up to 54% less than predictions of standard formulas. It should be considered that 28 kcal kg day and 23 kcal kg day be used when calculating energy requirements for paraplegics and quadriplegics, respectively. For individuals mechanically ventilated with multiple trauma, it should be considered that 27 kcal kg body weight day could be used up to three days post injury. Conclusions: Twenty eight studies met the inclusion criteria. Twenty six of these studies were Level 3, Grade C, one was Level 1, and one was Level 2. Recommendations were based on studies with Level 3, Grade C evidence and epivir.

HE OVERWHELMING RECOMMENDATIONS FROM THE WOMEN FOCUSED on the need to increase outreach and education in the Latino community on reproductive health issues. Participants who were parents wanted more information to share with their children and spoke of the need for greater sexual education training for Latino youth. This is especially important and relevant for California now as it experiences an increase in Latino youth and as Latinos comprise almost half of all students in the state. Many participants also discussed the need for increased outreach and public service announcements that specifically target the Latino community to be aired on radio stations, television stations, bus bench ads and in community clinics.
