Additionally, peripheral and splanchnic vasodilatation with the resulting hyperdynamic circulatory state is typical of cirrhosis and portal hypertension. Cirrhosis can remain compensated for many years prior to the development of a decompensating event. Decompensated cirrhosis is marked by the development of any of the following complications: jaundice, variceal hemorrhage, ascites or encephalopathy. Jaundice results from hepatic insufficiency and, other than liver transplantation, there is no specific therapy for this complication. It is however important to recognize and treat superimposed entities e.g., alcoholic hepatitis, drug hepatotoxicity ; that may contribute to the development of jaundice. The other complications of cirrhosis occur as a consequence of portal hypertension and or hepatic insufficiency. Gastroesophageal varices result almost solely from portal hypertension, although the hyperdynamic circulation contributes to variceal growth and hemorrhage, for example, etoposide aml.

It is highly successful in ovarian and breast cancer treatments. It is a complicated molecule, the total synthesis of which was reported in 1994 and involved over 40 steps. It was not commercially viable. On the other hand, yew bark contains only 0.01% taxol, so supply is problematic. Production from cell cultures has been disappointing, although proprietary plant-cell fermentation technology recently developed may help. The best solution so far has been semi-synthesis. A precursor is obtained from yew needles and modified chemically to give taxol; this is not an entirely satisfactory solution and taxol stands out as a good example of a plant-derived drug that is difficult to supply Covello, 2003 ; . Another approach is the application of molecular biology and biotechnology tools. With better knowledge of the genes involved in the biosynthesis of plant-derived drugs, their production may be enhanced in plants, plant-cell cultures or alternative hosts. This metabolic engineering approach is an area of active research. For instance, Rodney Croteau and colleagues at Washington State University are tackling the taxol problem in this way.
