By Jeff Swiatek After his heart attack last spring, Steve Preo didn't know he would play a role in one of the largest and financially riskiest drug trials Eli Lilly and Co. has ever undertaken. Preo, an Indianapolis graphic designer, has volunteered along with 13, 000 other heart patients to test an experimental anti- clotting drug that Lilly is developing with partner Sankyo Co. Ltd of Japan. For Lilly and its Japanese partner, the trial amounts to a bet in the tens of millions of dollars that the drug can outperform Plavix and carry instant credibility with doctors when it comes on the market. That's expected to be in 2008, if all goes well. "There's definitely a risk associated with this, " said Dr. J. Anthony Ware, Lilly's team leader for prasugrel, the drug in the study. "You have to prove you're actually superior to a drug that does work. It's a higher threshold" for a new drug to meet. Lilly has done other large head-to-head tests of its drugs against competing brands. One was with Evista, its osteoporosis drug. But those tests typically came after the Lilly drug already was on the market. Lilly engaged in "a lot of discussion" internally before funding the prasugrel-Plavix trial, Ware said. One of the deciding factors was the realization that, as drug costs continue to go up, "Our customers . are going to insist new ; medicines be better than the standard of care, " he said.

The apparatus is designed to incise the earlobe and collect two samples of arterialised blood without contamination of the environment of the spacecraft. The apparatus comprises two units. The blood-sampling unit to which the vasodilated earlobe is clamped carries: i ; A fine scalpel blade attached to a spring-loaded plunger release of which produces an incision 2-3 mm depth ; in the earlobe ii ; Two heparinised capillary tubes, each of which can be brought sequentially in contact with the blood flowing from the incision, and, iii ; A pledge of cottonwool, which can be brought over the incision to stem the bleeding. The blood sampling unit is held in place by a headband similar to a communication headset. In initial tests of the prototype apparatus, analysis of the blood samples collected from a healthy subject yielded a PO2 93 mmHg; PCO2 40 mmHg; Sa O2 96.9% and pH 7.37.
