Oral ofloxacin has been investigated as a single agent in two well-designed clinical trials, and it is effective against both N. gonorrhoeae and C. trachomatis. Despite the results of these trials, lack of anaerobic coverage with ofloxacin is a concern; the addition of metronidazole to the treatment regimen provides this coverage. Preliminary data suggest that levofloxacin is as effective as ofloxacin and may be substituted; its single daily dosing makes it advantageous from a compliance perspective.

By Jim Arachne, Positive Living Centre TABLE 1: DHEA IMPROVES T4 COUNT, WEIGHT GAIN AND WELL-BEING. After 6 months using DHEA, women experienced the following statistically significant changes: Body weight kg ; Change in energy scale of 0-100 ; Physical functioning Cognitive functioning Emotional well-being Current health perception Change in T4's 16 women took 50mg DHEA every day for 6 months and were compared to 13 similar women taking placebo. All were on the same 3 anti-HIV drugs and had been for at least 10 months previously. Note the overall improvement in a range of factors ie the women reported feeling better, both physically and psychologically. Very interesting is the substantial rise in average T4 count. This increase in T4 count has not been repeated in trials in men. One small trial closely followed nine HIV-negative older men average age 63 years ; who took 50mg of DHEA every night for 20 weeks 6 ; . All of the men had low DHEA levels. They began the trial with a two week period on placebo. Compared to results on placebo the men had: No change in T cell numbers but T cell activity increased by 40% p .05 ; . Natural killer cell NK ; numbers increased by 22-37% at 18 to 20 weeks p .01 ; and NK killing ability increased 45% by 18 to 20 weeks p .01 ; B cells numbers showed no clear change but B cell activity increased by 62% after 12 weeks p .05 ; DHEA + 1.36 kg + 8 107 PLACEBO - 1.19kg - 12 - 18 -5 -7 -8 - 11 what the researchers termed Mental Health p 0.001 ; and Health Distress 0.004 ; scores 7 ; . The average DHEA level for people with HIV only was 6.6 micromols L, whereas people with co-infection had much lower levels of 2.1 p .01 ; Use of DHEA Supplements If you are interested in following up this information here are some steps to take: First, get a doctor to check your blood levels of DHEA. DHEAs DHEA sulphate ; is probably the best form to measure. If your levels are in the optimum range see below ; then its unlikely that you'll get any benefit from taking a DHEA supplement. However, if levels are below optimum then you can expect that supplementing with DHEA will be of benefit. There is a difference between the normal range and the optimum range. An American doctor, Dr Kaiser, who has prescribed DHEA extensively for his patients with HIV, recommends raising DHEA levels to optimum. He has found that the optimum DHEAs level for men is between 300 600 ug dL and for women its between 100 300 ug dL. Levels will and tricor.

Ciprofloxacin, ofloxacin, or levofloxacin ; have been used frequently in the treatment of gonorrhea because of their high efficacy, ready availability, and convenience as a single-dose, oral therapy. However, prevalence of fluoroquinolone resistance in Neisseria gonorrhoeae has been increasing and is becoming widespread in the United States, necessitating changes in treatment regimens. Beginning in 2000, fluoroquinolones were no longer recommended for gonorrhea treatment in persons who acquired their infections in Asia or the Pacific Islands including Hawaii in 2002, this recommendation was extended to California 2 ; . In 2004, CDC recommended that fluoroquinolones not be used in the United States to treat gonorrhea in men who have sex with men MSM ; 3 ; . This report, based on data from the Gonococcal Isolate Surveillance Project GISP ; , summarizes data on fluoroquinolone-resistant N. gonorrhoeae QRNG ; in heterosexual males and in MSM throughout the United States. This report also updates CDC's Sexually Transmitted Diseases Treatment Guidelines, 2006 4 ; regarding the treatment of infections caused by N. gonorrhoeae. On the basis of the most recent evidence, CDC no longer recommends the use of fluoroquinolones for the treatment of gonococcal infections and associated conditions such as pelvic inflammatory disease PID ; . Consequently, only one class of drugs, the cephalosporins, is still recommended and available for the treatment of gonorrhea. GISP is a CDC-sponsored sentinel surveillance system that has been monitoring antimicrobial susceptibilities of N. gonorrhoeae in the United States since 1986. Annually, GISP collects approximately 6, 000 urethral gonococcal isolates from males attending 26 to 30 sexually transmitted disease STD ; clinics throughout the country and provides national data to guide treatment. QRNG isolates demonstrate ciprofloxacin minimum inhibitory concentrations MICs ; of 1.0 g mL; isolates with intermediate resistance to fluoroquinolones demonstrate ciprofloxacin MICs of 0.1250.500 g mL. GISP began susceptibility testing for ciprofloxacin in 1990. Overall, QRNG prevalence remained 1% during 19902001 but increased to 2.2% in 2002, to 4.1% in 2003, and to 6.8% in 2004. In 2005, of 6, 199 isolates collected by GISP, 9.4% were resistant to ciprofloxacin, and during JanuaryJune 2006, 13.3% of 3, 005 isolates collected were resistant Figure ; 5 ; . Excluding isolates from Hawaii and California areas that discontinued fluoroquinolone treatment in 2000 and 2002, respectively ; , 6.1% and 8.6% of isolates were QRNG in 2005 and 2006, respectively. Intermediate resistance to ciprofloxacin has remained stable, ranging from 0.4% to 1.1% from 1990 to 2006 5 and flavoxate.

Institute of Chemistry, University of the Punjab, Lahore, Pakistan, and Applied Chemistry Research Centre, PCSIR Labs. Complex, Ferozepur Road, Lahore, Pakistan Received February 2005, accepted September 2005 Communicated by Prof. Dr. A. R. Shakoori Abstract: The present study was conducted for the analysis and comparison of selected residual antibiotics in broiler chicken available in local market. The broiler samples included muscle, kidney and liver. The quinolones included in this study were, oxolinic acid, nalidixic acid, flumequine, enrofloxacin, norfloxacin and ciprofloxacin. An assessment of variation of these analytes residues ; in these samples was made. The intertissue organ comparison within each analytical technique and intermethod comparison of results obtained by HPLC, UV spectroscopy and ion association complex techniques were made. TLC was used to separate and identify the quinolone residues. Infrared IR ; was also used for identification of the residues. HPLC with ODS column and UV detector and UV visible spectroscopy were used for quantification of the residues. Oxolinic acid, nalidixic acid and norfloxacin residues were absent in all the samples. The "ciprofloxacinbrilliant blue G" ion association complex was used for ciprofloxacin determination using ion association complex technique. Good compatibility of the spectrophotometric results was found with those of high pressure liquid chromatography. Keywords: Maximum Residue Limits MRL's ; , Ion Association Complex Technique IAC ; and HPLC and urispas. Table 1. Baseline characteristics of 412 participants showing men and women separately and combined. Results are presented as mean SD. It is a sterile liquid formulation of a purified neurotoxin that acts at the neuromuscular junction to produce flaccid paralysis. Myobloc was approved by the FDA for the treatment of patients with cervical dystonia to reduce the severity of abnormal head position and neck pain. Myobloc was launched in the United States in December 2000 and Neurobloc was launched in the EU in March 2001. U.S. revenue for Myobloc amounted to $17.5 million for 2002. Frova Elan licensed exclusive North American sales and distribution rights for Frova in October 1998 from Vernalis Group, plc. Frova is a 5HT1B 1D agonist used as an anti-migraine therapy. In November 2001, the FDA approved Frova for the acute treatment of migraine. In March 2002, Elan and UCB Pharma, Inc. ``UCB'' ; entered into an agreement to co-promote Frova. The companies launched Frova during the second quarter of 2002. Elan markets, for example, ofloxacin solubility.

However, some of the new fluoroquinolones show considerable promise as oral agents for the treatment of gonorrhoea. Their use is contraindicated in pregnancy and they are not recommended for use in children and adolescents, although ciprofloxacin has been licensed in Denmark for the single-dose prophylaxis of meningococcal disease in children. The in vitro activity of individual fluoroquinolones against N. gonorrhoeae varies considerably. There is some evidence of increased minimal inhibitory concentrations in strains isolated after treatment with less active agents. Ciprofloxacin is considered to be the agent with the greatest activity against N. gonorrhoeae. Quinolone-resistant N. gonorrhoeae QRNG ; has become common in parts of Asia and the Pacific. In 1996 the proportions of quinolone-resistant gonococci reported in these areas ranged from less than 1% in New Zealand to 15% in the Republic of Korea, 24% in Hong Kong Special Administrative Region of China, 53% in Cambodia and 66% in the Philippines. In the USA, QRNG is becoming increasingly common in western regions. Quinolones are no longer recommended for the treatment of gonorrhoea in the State of Hawaii, and are to be used cautiously in California.18 Resistance of N. Gonorrhoeae to quinolones will continue to spread across the globe. It is imperative that surveillance for antimicrobial resistance be strengthened in order to guide treatment recommendations. Experience in the treatment of chlamydial infection with fluoroquinolones is limited. Of the currently studied agents, foloxacin has the greatest potential when given as 300 mg twice daily for seven days. This is effective against both gonorrhoea and.

CYP2D6 52 ; . Amitriptyline is frequently prescribed in Canada. Clinical studies have shown substantial increases in plasma concentrations of tricyclic antidepressants and associated signs of toxicity when certain SSRIs fluoxetine, paroxetine ; are coadministered 53, 54 ; . Other antidepressants without CYP2D6 inhibitory activity are nefazodone a potent CYP3A4 inhibitor ; and fluvoxamine a potent CYP1A2 inhibitor ; . CYP2C9 The CYP2C family of enzymes is represented in humans predominantly by four drug-metabolizing members: CYP2C8, CYP2C9, CYP2C18 and CYP2C19 55 ; . While this family is responsible for the metabolism of fewer drugs than CYP3A4 and CYP2D6 enzymes, one member CYP2C9 ; is particularly important because it metabolizes warfarin, phenytoin, and nonsteroidal anti-inflammatory drugs Table 3 ; . CYP2C9 is an inducible polymorphic enzyme. The frequency of CYP2C9 has been estimated at approximately 1% in whites, about 0.1% in African Americans and less than 0.1% in Asians. Warfarin is an oral anticoagulant that is increasingly used for the prevention of cardioembolic stroke and thromboembolism. While the average daily dose of warfarin is approximately 5 mg, there is tremendous variability in daily warfarin requirements, ranging from 0.5 mg up to 60 mg. Because warfarin has a narrow therapeutic index a small change in plasma drug concentrations can substantially alter drug effects ; , reduced level, from an inducer or enhanced concentration from an inhibitor is a common cause of adverse drug events during chronic warfarin therapy 56-59 ; . Phenytoin is a widely used anticonvulsant for the treatment of generalized tonic-clonic seizures and complex partial seizures. At clinical doses, plasma concentrations of phenytoin can be sufficiently high to saturate CYP2C9mediated drug metabolism producing nonlinear pharmacokinetics. A small increase in the dose of phenytoin can produce a markedly disproportionate increase in plasma drug concentrations. In addition, phenytoin has a narrow therapeutic index. Thus, a minor alteration in CYP2C9 metabolic activity by an inducer or inhibitor of this enzyme can produce untoward changes in clinical outcome 60-62 ; . CYP1A2 CYP1A2 plays a primary role in the metabolism of theophylline, an antiasthmatic medication. Theophylline belongs to a group of drugs in which plasma concentrations are routinely clinically measured and the dosage is adjusted so that the concentration stays within a narrow effective and safe range. Cigarette smoking induces CYP1A2 and reduces the plasma concentration and efficacy of theophylline 63 ; . Ciprofloxacin, cimetidine, and the SSRI fluvoxamine are inhibitors of CYP1A2, and can excessively increase plasma theophylline concentrations and the risk of toxicity, leading to cardiac arrhythmias and seizures 6467 and luvox and ofloxacin. MEN'S HEALTH IN A CONTEXTUAL PERSPECTIVE S.I. Andersson. Department of Psychology, Lund University, Lund, Sweden Background: More traditional ways of measuring health include use of mortality and morbidity figures in population studies and measures of functional and physiological status, In the 70s, measures of health status concerned both positive and negative aspects of health, although the instruments involved were often criticized for not including social and emotional aspects of well-being. Today, there appears to be some degree of consensus regarding the minimum set of physical, psychological, social, and both the more global and the more specific components to be included in measuring quality of life in health care. In the present study it is argued that the fragmentation often represented by a mixture of separate questions concerning biomedical, psychological and social cultural factors is a far from adequate approach to studying the health of women as well as men. Methods: The paper evaluates the use of both global and situationally related measures for assessing biopsychosocial problems such as those of pain, fatigue, muscle weakness, and sexuality ; as perceived by various groups of men - patients as well as controls - the numbers of subjects in already published and in ongoing studies presently exceeding 200. For each type of problems considered, subjects were to indicate its occurrence or non-occurrence yes no ; during the past week, and were also to select what they adjudged to be the three types of problems affecting them most strongly, describing in writing ; for each of these a typical situation in which it occurred. Results: A general finding is that an adequate understanding of experiences of illness requires access to contexts that are not typically regarded as being either objective or free of moral implications. This contrasts with what is frequently aimed at in biomedicine. Also, the traditional biopsychosocial model, though presented as many-sided and all-round, provides no integrated model for the study of health, but instead leaves the biological, psychological and social factors involved within a loose and unintegrated explanatory framework Conclusion: A contextual framework, such as outlined in the present study, can provide an understanding of more global factors that can enable interventions to become more effective. It is very crucial to remember that all pharmaceutical medications, although they can be beneficial, can cause possible adverse reactions. A patient with atypical clinical features, should prompt the search for an alternative diagnosis or supplemental support for a diagnosis of MS. Spinal cord MRI may be of considerable diagnostic value in these groups, of whom a significant proportion with MS will have spinal cord lesions. Spinal cord lesions also help distinguish between inflammatory and age-related or ischemic pathology in cases where brain MRI abnormalities are nonspecific Case 2 ; . Negative oligoclonal bands in established MS does occur, but are an unusual finding and should prompt further evaluation. Where access to paraclinical testing is limited, the diagnosis requires clinical confirmation of dissemination both in time and space and increased vigilance in screening for alternative diagnoses. POSSIBLE MS A significant number of patients will fulfil some but not all of the diagnostic criteria and, in these patients, a definitive diagnosis cannot be made. A discussion with the patient about the prognosis may be informed by results of paraclinical tests.
One of the biggest co-marketing initiatives in recent times is the just concluded deal between Ranbaxy for it's once a day version of the Cephalosporins, Ciprofloxacin and Ofloxacin. For the former, it has entered into a tripartite deal with the top players in the domestic market, Cipla and Glaxo India. While for the latter, it would be co-marketing the molecule with Zydus Cadila Healthcare, a leading player in that segment. In licensing of new generation molecules by leading overseas players is a key plank on which Nicholas Piramal has relied over the years. Recently, Wockhardt has emulated this move by in-licensing Bayer AG's Acarbose and Methocobal from Eisai Company, Japan to reinforce its anti diabetes range. Stadmed, Kolkatta is all set to launch a slew of new generation anti-diabetic, cardiovascular and neuro psychiatric drugs. The company is negotiating with an Australian major to procure EMRs in India for the latter's product range. It also plans to introduce special dietary supplements for people with chronic symptoms of arthritis and other diseases. A separate marketing division is being created for the same and the focus would be on the markets of North and South India apart from the East.
