The proposed paragraph on Reduced sensitivity of viral neuraminidase has been updated to reflect the findings in children from 1 to 12 years of age and to highlight the risk of emergence of resistance in younger children. There has been no evidence for emergence of drug resistance associated with the use of Tamiflu in clinical studies conducted to date in post exposure 7 days ; , post exposure within household groups 10 days ; and seasonal 42 days ; prevention of influenza. The risk of emergence of drug resistance in clinical use in the treatment of influenza has been extensively examined. In all clinical studies in naturally acquired infection, 0.32% 4 1245 ; of adults and adolescents and 4.1% 19 464, range 0-19% in individual studies19 464 ; of children aged 1-12 were found to transiently carry influenza virus with decreased neuraminidase susceptibility to oseltamivir carboxylate. The emergence of resistance may be higher in young children and in children who had immunosuppression or who were under-exposed to oseltamivir. Patients carrying resistant virus cleared it normally and showed no clinical deterioration. Rare cases of oseltamivir-resistant virus strains in patients who were not exposed to oseltamivir have been reported. All resistant genotypes are disadvantaged compared to the corresponding wild-type isolate and are likely to be less contagious in man. Thus far, there is no evidence for resistance in influenza B in vitro or in clinical trials. In clinical studies in naturally acquired infection, 0.34 % 4 1177 ; of adults and adolescents and 4.5 % 17 374 ; of children aged one to 12 years were found to transiently carry influenza A virus with decreased neuraminidase susceptibility to oseltamivir carboxylate. Neuraminidase from influenza B with reduced sensitivity has not been observed either in cell culture or in clinical studies. Cross resistance between zanamivir-resistant influenza mutants and oseltamivir-resistant influenza mutants has been observed in vitro. Insufficient information is available to fully characterise the risk of emergence of oseltamivir resistance and cross-resistance in clinical use. The findings of WV16193 have been reported as a paragraph on post-exposure prevention in adults, adolescents and children between 1 and 12 as this study is not specifically a paediatric study. It can therefore not be considered only for children between 1 and 12 years of age despite the scope of this variation. The proposed paragraph has been updated as follows: The efficacy of oseltamivir in preventing naturally occurring influenza illness has been demonstrated in a post-exposure prevention study in households that included adults, adolescents, and children aged 1 to 12 years, both as index cases and as family contacts. The primary efficacy parameter for this study was the incidence of laboratory-confirmed clinical influenza in the households. The oseltamivir prophylaxis lasted for 10 days. In the total population, there was a reduction in the incidence of laboratory-confirmed clinical influenza in households from 20% 27 136 ; in the group not receiving prevention to 7% 10 135 ; in the group receiving prevention 62.7% reduction, [95% CI 26.0-81.2]; p 0.0042 ; . In households of influenza infected index cases, there was a significant reduction in the incidence of influenza from 26% 23 89 ; in the group not receiving prevention to 11% 9 84 ; in the group receiving prevention . According to subgroup analysis in children at 1-12 years of age, the incidence of laboratory-confirmed clinical influenza among children was significantly reduced from 19 % 21 111 ; in the group not receiving prevention to 7 % 7 104 ; in the group receiving it. Among children who were not already shedding virus at baseline, the incidence of laboratory-confirmed clinical influenza was reduced from 21 % 15 70 ; the group not receiving prevention to 4 % 2 the group receiving prevention p 0.0206 ; . The NNT for the total paediatric population was 9 95 % CI 7-24 ; and 8 95 % CI 6, upper limit not estimable ; in the whole population ITT ; and in paediatric contacts of infected index cases ITTII ; respectively, for instance, pharmacist!

SALT MARKETING The study surveyed salt markets and traders in the country. While 93% of the production is sold within the country, some also goes to other countries, particularly Niger, Togo, and Burkina Faso. About 30% of the domestic salt is consumed in the Accra region. The preferred type of salt is "sea white" or "sea grey, " which together comprise 93% of the market. Less than 2% is rock salt. The report charts the salt traders and their source of salt by region. However, 45% of the traders did not know where their salt came from. About half of the salt traders buy from retailers, indicating that yet another layer exists between producer and consumer. The frequency of salt purchases is affected by limited working capital and limited salt demand. Eighty-one percent of the salt traders sell small amounts. Most sales take place in the market place but some also occur at the trader's residence. Only 5% of the traders interviewed were members of any trade association, the rest operating independently. Ninety-six percent of the salt traders had no knowledge of iodized salt, and the rest did not know its purpose. Some salt is imported, almost all from the United Kingdom, the rest from Germany and China. The amount imported fluctuates widely, for example, 50 metric tons in 1992, but only 2 metric tons in 1991. Ghana exports salt, in amounts ranging from 31, 000 to 75, 000 metric tons from 1987 to 1992. The major destinations were Niger, Burkina Faso, Togo, Nigeria, Mali, the Ivory Coast, Benin, and the Congo. The exports trade is not handled directly by salt producers but by others. Of the traders interviewed, 96% were females. Most traders deal in small quantities of 3-30 kilograms, purchased every week, two weeks, or month. The study notes that packaging of iodized salt should be kept to small sizes since the present marketing situation is not likely to change. IODIZED OIL ADMINISTRATION The survey showed that iodine deficiency was very severe in the upper east region. In 1991, UNICEF donated iodized oil capsules that were distributed in the field by a special team formed in the Nutrition Division headquarters. Coverage was small, the operation was expensive and the use of special task teams could not be sustained because it was not a normal routine of the Ministry of Health. For these reasons, an alternate system was adopted in 1993 to ensure that iodized oil administration was integrated into existing Ministry of Health programs. In this project iodized oil capsules were distributed to district health medical teams, who were encouraged to deliver capsules at outpatient departments, outreach services, child welfare clinics, antenatal clinics, school health clinics, and home visits. The objective was to give a full dose of iodized oil to all individuals, aged 1-45 years. Problems in this program included difficulty in recording capsule administration, an uneven supply of iodized oil capsules and demand outstripping supply. Follow-up of this program is needed. FURTHER PLANS The conference reported on progress towards salt iodization. So far, salt producers have been sensitized to the iodization program, the needs assessment for production and iodization equipment has been completed, and orders placed. The Standards Board has produced detailed and zestril.

Thompson 1995: 1276 ; defines a support system as an organised group of individuals who encourage or assist. In this study a support system will mean a group of people who come together with the aim of assisting diabetic patients with their daily life skills. According to findings of Chen-Yen and Fensle 1996: 468 ; , Gowers, Jones, Kiana, North and Price 1995: 995 ; , Keller 1998 ; in Cleaver and Pallourios 1994: 176 ; and Ruth 1999: 420 ; , the patient's type of support system does have an influence on his her adherence to the diabetic regimen. Individuals are more likely to adhere to their diabetic health regimen if the quality of their interaction and relationship with their health providers is good and if the individuals have good social support. Patients whose families are supportive take their medication correctly, follow a diabetic diet such as low fat, high carbohydrate and fruit, and participate in various exercises such as walking, running, rugby and swimming Gowers et al 1995: 995 ; . Keller 1998 ; also maintains that the diabetic patient's adjustment to the disorder depends largely on family support, and that negative patterns of interaction within the family affect the adherence to the diabetic regimen, and can lead to lack of control and even to attacks of keto acidosis Cleaver & Pallourious 1994: 176 ; . Brown and Segal 1996: 903 ; indicate that non-compliance rates are higher among patients with symptomatic chronic diseases, requiring long term or lifetime medication therapy. Brown and Segal further indicate that failure to "comply" with prescribed medication regimens does not mean that the patient goes untreated. It is conceivable that patients who are labelled as non compliant by the medical establishment may actually be managing their condition in ways most consistent with their models of illness, for example, a person may not be "compliant" with his her prescribed regimen but be perfectly "compliant" with some alternative regimen. In fact, many ethnic minority patients make simultaneous use of both the formal and informal medical care systems. It can therefore be deduced that quality interaction between health providers and diabetics and well-informed family members who support their diabetic relatives, contribute towards diabetic patient compliance to treatment regimen. 2.4.2 Some of the feelings of frustration.

Yperlipidemia and the atherosclerotic conditions that result from it are well recognized as major contributors to coronary heart disease CHD ; . Fortunately, several large-scale clinical trials have shown that there are effective treatments that can substantially lower atherogenic lipid levels and thereby reduce the risk of CHD mortality and morbidity. However, duplication of these dramatic trial results can be negatively affected in "real life" clinical practice by an important issue: compliance. No medications will work if patients do not take them. Unfortunately, patients who need lipid-lowering therapy are likely to need it long-term, perhaps for a lifetime. Yet, many do not adhere to the prescribed medication regimen. This article reviews some major studies of compliance for lipid-lowering drugs. The reasons why patients do not take them as prescribed vary: poor education, lack of understanding, cost, provider indifference, and others. Achieving compliance requires a multifaceted approach. It can be enhanced by encouraging patients to talk openly about their medication habits and by convincing them of the long-term benefits of reaching and maintaining target low-density lipoprotein cholesterol levels. Although more studies focusing on compliance specifically regarding CHD are needed, the current literature does provide some guidance. Arch Fam Med. 2000; 9: 1169-1175.
